Breast Augmentation
Incision Sites and Placement of the Implants
Armpit Incision (transaxillary site):
The transaxillary approach to breast augmentation refers to the incision being placed in the skin folds of the armpit. No actual incision is done on the breasts themselves. The scar is minimal or hardly none at all. It is only visible when the arms are held straight up over the head.
Naval Incision (TUBA):
This approach is fairly new having been developed in the 90s. It is slowly gaining popularity with women who opt for certain types of implants. There is virtually no scar and access is gained through the naval with endoscopic methods.
Under the Breast Incision (imframammary site):
This method is by far the most widely used and popular method for placing a breast implant. The incision is placed below the crease of the breast which makes it easier for breast re-augmentation. One drawback to this method is that scarring is visible when the arms are held straight over the head.
The cosmetic surgeon will determine the best incision site for you after you choose which type of implant you desire and the desired results you are trying to achieve. Not every incision site produces the same result in terms of recovery and discomfort.
Placement of the Implants:
There are three placement sites of the implants generally used by the cosmetic surgeon performing breast augmentation. They are: Under the Muscle Implant, Above the Muscle Implant, and Partial Placement in Between. The placement location of the breast implant is determine by the cosmetic surgeon and varies with the selection of the implant. Each placement site has its own advantages and disadvantages which your cosmetic surgeon will discuss during the consultation with you.
Typically a breast implant can be placed either under the breast tissue (over the muscle) or under the breast muscle (below the breast tissue). Combined with utlizing the minimum scarring approach, which entails making the incisions along the armpit (transaxillary incision), along the areola, or under the breast at the crease. It will depend on your unique situation and the cosmetic surgeon's preference.
Here are some questions to ask your cosmetic surgeon:
Saline or silicone gel implant? What are the advantages and disadvantages?
What size and shape will give me the proportionate look with the rest of my figure? Should I opt for fixed or adjustable size and what are those advantages and disadvantages?
Will pregnancy and childbirth have any impact on my breast augmentation?
Possible Risks:
As with all surgery, there are risks to undergoing breast augmentation. Some of the risks include infection and excessive bleeding, mainly in the implant pocket. Some of the other problems can develop after surgery, and they can be as follows: implant deflation, calcification in the pocket, or asymmetry, which may be a result of implant slippage. One of the rare complication that occurs is with the silicone gel. It is called capsular contracture in which the scar tissue forms around the implant tightens within the pocket causing the breast to feel hard. Your cosmetic surgeon will discuss in detail all of the risks and complications with you during your breast augmentation consultation.
Recovery:
Because of the local anesthesia, recovery is fast. As expected, you will be sore and tired for a couple of days following the procedure. Your cosmetic surgeon will keep your covered with strong pain medication during your recovery time. But most of the discomfort will go away in 24 to 48 hours.
At first, you will have a gauge dressing that will need to be changed but after some time, you will be given a special bra. Swelling in your breasts may take three to five weeks to cease, and your stiches, if any, should come out in ten to twelve days. At any time during the recovery period, your cosmetic surgeon will be available 24 hours a day/7 days a week to answer any questions or concerns you may have.
